Need Help For SEO Analysis

I have a webpage, which got ranked on first position on google, but since 1 year i lost rank.. Now its on 39 SERP.. don`t know whats wrong with my page. please guide me which part of SEO is missing or need to improve?

Rahul Gupta: +Moses Aaron Your site create session Id that need to be fixed, also I’ve noticed that your site doesn’t show character count (can check in GWT), one last i.e. 404 page not found in you site, its redirecting to home page. Its looks like you have redirected the link juice effect of 404 error pages, is stronger than the negative impact of redirecting 404 pages! So work more on analyzing the technical issues and fix them to get ranked.
