Ghost spam!

Google Analytics showing traffic from the link which is not there on the website? However, little google research shows that it could be a case of ghost spam!! what could be the probable reasons and how to fix this? Any help will be appreciated! :)

Divyanshu Dubey: some people have recommended to add segments!!
Jim Munro: It does not affect your site so you can safely ignore it. The entries only hit your Analytics, they do not call pages from your site.
Divyanshu Dubey: Is there any way to remove such links/data from analytics?
