How do spam links to the site appear?

Just curious. How do spam links to the site appear? I mean without site owner actively involved in creating them?My disavow file is huge and it keeps growing.I know I am not involved in any spam link building and I do not think it is a "black hat" competitors SEO tactics.

Buth Main: If you didn`t create the spam links, you don`t have to disavow these links. Also it is a risky way, because there are a few SEOs who can identify really spam links. "SEO tools" can`t help you.
