Pages not being indexed under any keywords

Launched my site on 3/22 it IS indexed. My main keywords are Phoenix Website Design. Twice it has shown up on page 17. most of the time it is not indexed under any of the keywords I`ve focused on. What Have I done wrong?
I have done all of the steps I know of to optimize the page. This isn`t my first rodeo. Any Ideas?
